NEW HIRE ELIGIBILITY SAMPLE

  • Hire date – 03/15/2023
  • New Hire Enrollment Period – Starts on 03/15/2023   Ends on 04/14/2023
  • Coverage Effective Date – 05/01/2023

SPECIAL ENROLLMENT PERIOD ELIGIBILITY SAMPLE

Employee did not enroll during “New Hire Eligibility Period” because of existing coverage. Employee is now losing their health insurance, and needs to enroll.

  • Date of loss 05/31/2023
  • Notification of QLE sent to HR on 05/20/2023
  • Effective date of Coverage 6/1/2023

ENROLLMENT/ WAIVING INSTRUCTIONS:

iTech is happy to share that we use ADP Workforce Now as our payroll provider and Benefit Enrollment Portal. This creates a secure, paperless, onboarding process, and virtually eliminates the chance of errors as all information is entered directly by the employee.

New Hires have exactly 30 days starting on the date of hire to submit benefit elections and or waivers through the ADP WorkforceNow Portal.

KEY POINTS:

  • The “New Hire Eligibility Period ” starts on the date of hire and ends 30 days after.
  • Elected plans are effective on the 1st of the month following 30 days of employment.
  • iTech’s benefit year goes from March to February
  • Annual Open Enrollment Period – Mid February, with benefits effective March 1
  • Special Enrollment Period – Triggered by Qualified Life Event (QLE). Request for coverage must be made within 30 days of QLE.

RETIREMENT

iTech offers employees the opportunity to save for their retirement with the iTech Solutions 401(k) plan managed by Voya®.

ELIGIBILITY

Employees may begin contributing to and/or roll over funds any day after completing 6 months of employment.

ENROLLMENT

Voya sends out eligibility documentation to employees once they are getting close to their eligibility date. However, employees can process their enrollment online as soon as they complete 6 months of employment.
